Warwick Community Ambulance Association (WCAA) is committed to protecting your personal health information (PHI). We are required by law to maintain the privacy of PHI that could reasonably be used to identify you and to provide you with this Notice of Privacy Practices explaining our legal duties and privacy practices.
We respect your privacy and treat all healthcare information about our patients with care under strict confidentiality policies.
Uses and Disclosures of PHI Without Your Authorization
WCAA may use or disclose your PHI without your authorization for the following purposes:
- Treatment: This includes sharing PHI with medical personnel involved in your care and treatment.
- Payment: Activities required to get reimbursed for the services provided to you.

We may contact you about scheduled transport, other services, or fundraising activities. You have the right to opt out of receiving fundraising communications.
Other Uses and Disclosures Without Authorization
WCAA may also disclose your PHI without your authorization for purposes including:

- Organ Donation: Facilitating organ, eye, or tissue donations.

Uses and Disclosures of PHI Requiring Your Written Consent
Any other use or disclosure of your PHI will only be made with your written authorization. This includes:
- Psychotherapy notes, unless used for treatment, payment, or healthcare operations.
- PHI used for marketing when compensation is received.
- Sale of your PHI.
You may revoke your authorization at any time in writing.
Your Rights Regarding Your PHI:
- Right to Access and Inspect: You may request to inspect and obtain a copy of your PHI. We may charge a reasonable fee, and certain denials may be appealed.
- Right to Amend: You may request an amendment to your PHI if you believe it to be incorrect.
- Right to an Accounting of Disclosures: You can request a list of certain disclosures made within six years prior to your request.
- Right to Request Restrictions: You may request restrictions on the use and disclosure of your PHI, though we may not be required to comply.
- Right to Breach Notification: You will be notified of any breach of your unsecured PHI.
- Right to Confidential Communications: You may request that we communicate with you in a specific manner or at a specific location.
Warwick Community Ambulance Association reserves the right to change the terms of this Notice at any time. Changes will apply to all PHI we maintain. The latest version of this Notice will be available at our facilities and on our website.
Your Legal Rights and Complaints
If you believe your privacy rights have been violated, you may file a complaint with WCAA or the Secretary of the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
